Please complete application at least 3 weeks before the show. Late applications will be charged a $25 late fee and very last-minute applications may not be considered.

IMPORTANT! Permits must be completely filled out, including menu, sketch and Workmans Comp Affidavit.
Allergen Training: MA Food Code requires all establishments providing food to the public to have a Person In Charge with an allergen awareness certificate. The training for this certificate is only $10 and can easily be obtained by watching an online video at www.berkshireahec.org/programs/ma-food-allergen-training/.
